○予約・問診に関して/ Regarding Appointments and Questionnaires.

Q:在校生で予約していない、予約を忘れてしまった For current students who couldn’t make an appointment or forgot to make an appointment.	健診最終日4/11（金）までは予約可能です。MySophiaやWebPilotiを確認のうえ、予約を取ってください。なお4/5以降に予約をした場合は、問診票の回答が出来ていないので健診当日に問診票へ記入をお願いします。 Appointments for medical checkup are available until the final date, April 11 (Fri). Please check MySophia or WebPiloti and make your appointment accordingly. If you make an appointment on or after April 5 (Sat), you will not be able to fill out the medical questionnaire in advance. Please complete the questionnaire on the day of the medical checkup.
Q:新入生と在校生で問診票に答えていない、問診入力期間が終わってしまった For those who have not answered or the submission deadline has passed.	新入生：4/7（月）まで入力が可能です。もし入力できなかったら健診当日に紙の問診票への記入をお願いします。在校生：4/4（金）まで入力が可能です。もし入力できなかったら予約のみ行い、健診当日に紙の問診票への記入をお願いします。 For new students: You can fill out the medical questionnaire until April 7 (Mon). If you are unable to do so, please fill out the paper questionnaire on the day of the medical checkup. For current students: You can fill out the medical questionnaire until April 4 (Fri). If you are unable to do so, please make an appointment and fill out the paper questionnaire on the day of the medical checkup.
Q:期間中に健診を受診できない For those who are unable to undergo the medical checkup during the designated period.	学外の大学指定健診機関（恵比寿健診センター）で健診をうけることが可能です。自己負担(2,750円)になります。詳細はMySophiaの「2025 年度健康診断学外指定健診機関の案内」をご確認ください。学外の大学指定健診機関での受診は、一番早くて女子は4/23（水）、男子は4/24（木）となります。 It is possible to undergo a medical checkup at an off-campus university-designated medical institution (Ebisu Kenshin Center). The cost will be (2,750 yen). For details, please refer to “2025 Medical Checkup for Students at Designated Medical Institution” on MySophia. The earliest available dates for the off-campus medical checkup are April 23 (Wed) for females and April 24 (Thu) for males.
Q:体調不良・コロナなどの感染などで行けそうにない For those who are unable to undergo due to illness or infection such as COVID:	当日体調不良の場合や感染症（コロナなど）の場合は、無理をせずに体調が回復したら学外の大学指定健診機関での受診をしてください。受診費用は自己負担になります。詳しくはMySophiaをご確認ください。 If you are feeling unwell or have contracted an infectious disease (such as COVID-19) on the day of the medical checkup, please do not push yourself, and instead, undergo the checkup at an off-campus university-designated medical institution once your health has recovered. The cost of the checkup will be at your own expense. For more details, please refer to MySophia.
Q:予約時間に間に合わない For those who cannot make it to your appointment time:	新入生：性別の合う他の日時で受診可能です。在校生：➀健診日時予約確定時のメールから「キャンセル」いただき、他の日時に変更してください。 ②予約時間が過ぎていたら、新たに健診予約入力をしてください。 New students: You can undergo on another date that matches your sex. Current students: Please cancel your appointment from the confirmation email and reschedule another date. If your appointment time has already passed, please make a new appointment for the medical checkup.
Q:希望の予約日時が埋まっていて予約できない For those who cannot make an appointment because your desired date and time are fully booked.	他の空いている日にち・時間帯で、性別の合う他の希望の日時に来てください。 Please come on another available date and time that match your sex.

○健診に関して Regarding Day of Medical Checkup.

Q:当日何をもって行けばいいか What to bring on the day of the medical checkup:	① 学生証（健康診断受付に必要） ② 検尿容器（検査当日採尿しておくこと） ③ 無地のTシャツ（レントゲン撮影時に使用します） 1. Student ID (required for medical checkup registration) 2. Urine sample container (please collect a urine sample before coming on the day of the checkup) 3. Plain T-shirt (to be worn during the chest X-ray examination)
Q:健康診断で忘れ物をした I forgot something at the medical checkup.	2号館1階の環境整備グループ・遺失物担当にご確認ください。 Please check with the Office of Environment and Facility Management on the 1st floor of Building 2.
Q:検査項目は？ Medical checkup examination items:	問診、尿検査、身長、体重、血圧、内科診察、胸部X線撮影 ※当日の健診結果による追加項目（心電図）がある場合があります。 ※看護学科は上記以外にも検査があります。 Medical questionnaire, urine test, height, weight measurement, blood pressure, physical examination by internal medicine, Chest X-ray ※Additional items (e.g., electrocardiogram) may be conducted depending on the results of the medical checkup on the day. Students in the nursing department may have additional tests beyond the above items.

Q:検尿の容器はどこでもらえるか
Where can I get the urine sample container?

在校生でガイダンスがある学科は、そのガイダンスで配布されます。
また、健診最終日までは、2号館1Ｆのキャリアセンター前と、ホフマンホールのウェルネスセンターの前にあります。
For current students: In departments with guidance sessions, containers will be distributed during those sessions. Additionally, until the final day of the medical checkup, containers are available in front of the Career Center on the 1st floor of Building 2 and in front of the Health and Wellbeing Center Health Services in Hoffmann Hall.

Q:健診期間が生理中だがどうすればいいか
What should I do if I am on my period during the medical checkup?

4/6（日）から4/11（金）まで健診期間。受診日以降であれば、検尿のみ、健診期間中の他の日に提出することが可能です（学生証要）。その際は、受診日に受付でその旨お申し出ください。生理中であっても必ず提出してください。
※もし尿潜血が認められれば5月に再検査（5/8・14・19いずれか）のご案内をします。
The medical checkup period is from April 6 (Sun) to April 11 (Fri). you can submit a urine sample on any day after your appointment during the medical checkup period (It cannot be submitted before your appointment). Student ID required. Please inform the reception of your situation on the day of your appointment. Even if you are on your period, you must submit the sample.
*If occult blood is detected in the urine, you will be instructed to undergo a retest in May (on one of the following dates: May 8, 14, or 19).

○健診結果に関して Regarding Medical checkup result.

Q:外部で受けた健診結果の提出でも大丈夫か Is it acceptable to submit medical checkup results obtained from an external medical institution?	学生の皆さんの健康状態を把握したいので、原則すべての方にご受診をお願いしています。また、外部医療機関での健診結果を代替としてお預かりするということはしておりませんので、ご了承ください。(社会人の方で勤務先にて毎年健康診断を受診している方等は、継続的に健康管理されているという観点からご受診の判断はお任せします。) なお、大学で行われる4月の健康診断か、大学指定の学外指定健診機関で受診した健康診断のどちらかでないと、大学名の「健康診断証明書」は発行できませんのでご注意下さい。 We kindly ask all students to undergo a medical checkup, as we would like to monitor the health status of students. Thank you for your understanding but we do not accept medical checkup results from external medical institutions as a substitute. If you are currently employed and receive an annual medical checkup through your workplace, we consider that you are under continuous health management, so we leave the decision of whether to undergo the medical checkup to you. Please note that the medical checkup certificate under the university’s name cannot be issued unless you undergo the medical checkup conducted in April at the university or at the off-campus university-designated institution.
Q:就職活動のため、早く健康診断証明書がほしい I need a medical checkup certificate soon for job hunting.	4月の定期健康診断を受診し、特に所見が認められなければ5/19（月）から健康診断証明書の発行が可能です。提出先に状況をお伝えし、提出日の相談をしてください。 After undergoing the medical checkup in April, if there are particular findings, the medical checkup certificate can be issued from May 19 (Mon). Please inform the submission recipient of your situation and consult with them regarding the submission date.
Q:留学や奨学金申請のため、早く健康診断証明書がほしい（学内提出） I need a medical checkup certificate soon for study abroad or scholarship applications (for submission within the university).	担当部署であるグローバル教育センターもしくは学生センターに相談し、提出日の相談をしてください。 Please consult with either the Global Education Center or the Student Center, and discuss the submission date.
Q:いつ結果が出るのか When will the results be available?	検査結果が問題ない場合のみ、5/19（月）に、2号館4階の自動発行機にて証明書の発行が可能です。 If there are no finding with the test results, the certificate can be issued from May 19 (Mon), at the automatic issuing machine on the 4th floor of Building 2.
